Terms and Conditions Overview

There are some terms and conditions upon registering at Quality Dental: Worthing; you can request to view our policies in full if you wish, below is an overview; they are to ensure that the practice runs smoothly and that the patients receive a safe and consistent service.

This includes patient based policies as well; this is to ensure that we can maintain an efficient service for all our patients.

By signing and dating the box below you agree to our terms and conditions and practice polices.


Charges/Payments
Are due on the day of your appointment and you will be required to pay for your treatment in full before the date your treatment is due to be completed.

Deposits/Part Payments
You may be required to pay in full or partly by means of deposit prior to booking appointments for private treatments. This amount may vary depending on the cost of treatment, for a hygienist appointment this is usually a set fee of £20 (or in full when booking online) and is required before a booking is confirmed.

You may be required to pay a deposit for certain treatments to commence to secure the booking where third party or associated fees to treatment are present.

Late Cancellation/Failure to attend
Failure to provide correct notice or failure to notify can result in a “Flag” being applied to your records. As an NHS patient this must not exceed three instances within a two rolling years and could result in De-Registration.

If a new NHS patient misses their first appointment they will automatically loose access to NHS services at the practice.

If a deposit has been paid, you may lose this deposit to cover lost clinical time or be charged a set fee (normally your deposit) or £1 per minute for the duration of the appointment.

The above will apply where less than 24 Hours’ notice for General Dental Services and 48 Hours notice for Specialist Dental Services is given, the surgery will have complete discretion with regards to exceptional circumstances.

Removal from the practice
NHS patients must maintain visits based on their recall frequency and should ensure they are seen for an examination no less that at least once within a two year period. Failure to do so may result in removal as an NHS patient so the practice can offer this to someone on our waiting list.
